Eligibility Criteria
- Alberta resident
- Income below threshold (based on previous year’s tax return)
- Not receiving AISH or Income Support
Application Process
- Download the Application Form: AEHB3931
- Complete the Form: Fill in all sections, including income and household details.
- Sign the Declaration: Confirm your consent for CRA to verify your income.
- Submit: Mail or email the completed form to the Alberta Health Benefits Contact Centre.
