Eligibility Criteria
- Alberta resident
- Income below threshold (based on previous year’s tax return)
- Children up to 18 years old (18- or 19-year-olds qualify if living at home and attending high school)
Application Process
- Download the Application Form: AHC0208W
- Complete Sections A–D: Include personal, income, and consent information.
- Sign and Date: Both the applicant and spouse (if applicable) must sign.
- Submit: Mail the form to Alberta Health or apply through Alberta Blue Cross.
