Must be under 65, have contributed to CPP, and have a severe and prolonged disability.
Application Process
- Gather documents: You’ll need your SIN, medical information, and employment history.
- Complete Form ISP-1151: This is the main application form.
- Medical Report (ISP-2519): Must be completed by your doctor or nurse practitioner.
- Submit online via My Service Canada Account or mail the paper forms to Service Canada.
- Wait for decision: Processing can take several months. You’ll receive a letter with the decision.

Possible Outcomes & Appeal Process
Appeals Process
- Reconsideration Request: If denied, you must request a reconsideration within 90 days of the decision. Use Form ISP-1145.
- Submit by mail or online via My Service Canada Account (MSCA).
- New Review: A different Service Canada officer will review your case.
- Still Disagree? You can appeal to the Social Security Tribunal (SST) – General Division.
- Further Appeal: If needed, appeal to the SST – Appeal Division, and then to the Federal Court 1.
